    THE EFFECT OF TURNING DIRECTION ON LOWER EXTREMITY JOINT MOMENTS DURING FAST CUTTING MOVEMENT

    The purpose of this study was to investigate the effect of turning movement on the three-dimensional moments at the ankle, knee, and hip joints. Data were collected using video cameras and force plate. Eight male recreational basketball players were tested during fast running (4.5m/s) and when cutting to the right or left off (+60, +30, 0, -30, and -60). The inverse dynamics approach was used to integrate the body segment parameter, kinetic and force plate data, and to solve the resultant joint moments. Greater abduction moments of the ankle, adduction moments of the knee and external rotation and adduction moments of the hip were found compared to values for straight running.     Protection of cerebral microcirculation, mitochondrial function and electrocortical activity by small-volume resuscitation with terlipressin in a model of haemorrhagic shock

    Background During early treatment of haemorrhagic shock, cerebral perfusion pressure can be restored by small-volume resuscitation with vasopressors. Whether this therapy is improved with additional fluid remains unknown. We assessed the value of terlipressin and lactated Ringer’s solution (LR) on the early recovery of the microcirculation, tissue oxygenation, and mitochondrial and electrophysiological function in the rat cerebral cortex. Methods Animals treated with LR replacing three times (3x) the volume bled (n=26), terlipressin (n=27), terlipressin plus LR of 1x (n=26), 2x (n=16), or 3x (n=15) were compared with untreated (n=36) and sham-operated rats (n=17). In vivo confocal microscopy was used to assess cortical capillary perfusion, changes in tissue oxygen concentration, and mitochondrial membrane potential and redox state. Electrophysiological function was assessed by cortical somatosensory evoked potentials (SEPs), spinal cord dorsum potential, and peripheral electromyography. Results Compared with sham, the mean (SD) area of perfused vessels was lower in rats subjected to haemorrhagic shock: 82 (10)% vs. 38 (12)%; P<0.001) and impaired oxygen concentration, mitochondrial redox state (99±4 vs. 59±15 % of baseline; P<0.001), and SEPs (97±13% vs. 27±19% of baseline). Adminstration of terlipressin plus 1X or 2X LR was able to recover these measures, but terlipressin+3LR or 3LR alone were not as effective. Spinal cord dorsum potential was preserved in all groups, but no therapy protected electromyographic function. Conclusion Resuscitation from haemorrhagic shock using terlipressin with small-volume LR was superior to high-volume LR, with regard to cerebral microcirculation, and mitochondrial and electrophysiological function

    Long-Term Outcomes of Everolimus Therapy in De Novo Liver Transplantation: A Systematic Review and Meta-Analysis of Randomized Controlled Trials

    BACKGROUND: Risk of nephrotoxicity in liver transplant patients on calcineurin inhibitors (CnIs) is a concern. Several controlled trials reported benefit of everolimus (EVR) in minimizing this risk when combined with a reduced CnI dose. BACKGROUND: To systematically review the efficacy and safety of EVR, alone or with reduced CnI dose, as compared to CnI alone post-liver transplantation. METHODS: We searched MEDLINE, Scopus, and the Cochrane Library for randomized controlled trials comparing EVR- and CnI-based regimens post-liver transplantation. Assessment of studies and data extraction were undertaken independently. RESULTS: Eight studies were selected, describing 769 patients. Cockcroft-Gault GFR was higher at one (P = .05), 3, and 5 years (P = .030) in patients on EVR compared to those receiving CnI therapy. The composite endpoint of efficacy failure was similar between the 2 arms after 1, 3, and 5 years of study. More patients discontinued EVR due to adverse effects in 1 year; however, no difference was noted after 3 or 5 years. A higher rates of proteinuria, peripheral edema, and incisional hernia occurred in patients on EVR. CONCLUSIONS: The analysis confirms noninferiority of EVR and reduced CnI combination. Combination regimen resulted in better renal function compared to standard CnI therapy

    Hypothermia protects brain mitochondrial function from hypoxemia in a murine model of sepsis

    Sepsis is commonly associated with brain dysfunction, but the underlying mechanisms remain unclear, although mitochondrial dysfunction and microvascular abnormalities have been implicated. We therefore assessed whether cerebral mitochondrial dysfunction during systemic endotoxemia in mice increased mitochondrial sensitivity to a further bioenergetic insult (hyoxemia), and whether hypothermia could improve outcome. Mice (C57bl/6) were injected intraperitoneally with lipopolysaccharide (LPS) (5 mg/kg; n = 85) or saline (0.01 ml/g; n = 47). Six, 24 and 48h later, we used confocal imaging in vivo to assess cerebral mitochondrial redox potential and cortical oxygenation in response to changes in inspired oxygen. The fraction of inspired oxygen (FiO2) at which the cortical redox potential changed was compared between groups. In a subset of animals, spontaneous hypothermia was maintained or controlled hypothermia induced during imaging. Decreasing FiO2 resulted in a more reduced cerebral redox state around veins but preserved oxidation around arteries. This pattern appeared at a higher FiO2 in LPS-injected animals, suggesting an increased sensitivity of cortical mitochondria to hypoxemia. This increased sensitivity was accompanied by a decrease in cortical oxygenation, but was attenuated by hypothermia. These results suggest that systemic endotoxemia influences cortical oxygenation and mitochondrial function, and that therapeutic hypothermia can be protective

    Experimental Demonstration of Masking Phenomena between Competing Odorants via an Air Dilution Sensory Test

    To simulate the occurrence of masking phenomena with the aid of an air dilution sensory (ADS) test, two types of odorant mixtures were prepared: (1) M2 with two individual odorants [H2S and acetaldehyde (AA)] and (2) M6 with six individual odorants (H2S and five aldehydes). The test results derived for samples containing single individual odorants at a wide range of concentrations are initially used to define the empirical relationship between the dilution-to-threshold (D/T) ratio and odor intensity (OI) scaling. Based on these relationships, the D/T ratios were estimated for each odorant with the same intensity as the synthetic mixture. The relative contribution of each odorant to such mixture is then assessed by comparing the estimated and measured D/T values. This stepwise test confirmed the dominance of certain compounds at a given OI rating. In the case of M2, H2S showed sensitive detection at high OI range, while AA did so at low end. The pattern of a competing relationship is also seen consistently from M6 between AA (low) and iso-valeraldehyde (IA: high OI range). The overall results thus suggest that the masking phenomena between strong odorants should proceed under competing relationships, if released at the same time

    Persistent Pemphigus Vulgaris Showing Features of Tufted Hair Folliculitis

    Pemphigus vulgaris is an autoimmune blistering disease that commonly involves the scalp. Lesions of pemphigus vulgaris that persist on the scalp for a long period may be accompanied by tufted hair folliculitis. Only two previous accounts of tufted hair folliculitis developing in a lesion of pemphigus vulgaris have been reported. We report a 51-year-old-man with erosions and clusters of hair on the scalp. The scalp lesion had persisted for about 20 years. A histopathological examination of the skin lesion on the scalp revealed separation of the epidermis and clusters of several adjacent hair follicles. The patient was diagnosed with persistent pemphigus vulgaris of the scalp showing features of tufted hair folliculitis

    Context into text into context : marketing practice into theory ; marketing theory into practice

    Purpose - The purpose of this paper is to explore the dynamics of marketing practice and theory in arguing that much of the dislocation between strategy and practice is due to the inheritance and internalisation of often impractical but persistently dominant, tacit Cartesian assumptions. Design/methodology/approach - This paper uses case methodology to examine the marketing theory into practice/marketing practice into theory conundrum and explores: their separation (marketing theory and marketing practice); their flows (context to text to context: theory into practice/practice into theory); their symbiosis (the praxis of marketing); and the dynamic and static (in situ/in aspic) nature of their duality. This work is an exploratory empirical study undertaken in what is a very under-researched area. Findings - In this paper, marketing theory and marketing practice are recognised as occupying different epistemes. The lifeworld of marketing theorising appears as characterised by a relatively homogenous and mostly cognitive world dominated by rationality and empirical rigour. By contrast, the embodied practitioner inhabits a more highly segmented, fragmented, heterogeneous and frequently improvised landscape. Practical implications - The authors propose that the all-consuming clamour for reliance and relevance of theory to practice dictates that the form, function and philosophy of marketing must be co-created in the practical pragmatism of praxis. Praxis is practice informed by theory and theory informed by practice, a cyclical process of experiential, contextual learning. Originality/value - The paper appears to be the first to bring together Cartesian thought and the practice-theory divide in B2B marketing theory

    Global organization of metabolic fluxes in the bacterium, Escherichia coli

    Cellular metabolism, the integrated interconversion of thousands of metabolic substrates through enzyme-catalyzed biochemical reactions, is the most investigated complex intercellular web of molecular interactions. While the topological organization of individual reactions into metabolic networks is increasingly well understood, the principles governing their global functional utilization under different growth conditions pose many open questions. We implement a flux balance analysis of the E. coli MG1655 metabolism, finding that the network utilization is highly uneven: while most metabolic reactions have small fluxes, the metabolism's activity is dominated by several reactions with very high fluxes. E. coli responds to changes in growth conditions by reorganizing the rates of selected fluxes predominantly within this high flux backbone. The identified behavior likely represents a universal feature of metabolic activity in all cells, with potential implications to metabolic engineering.Comment: 15 pages 4 figure
